Notice of Privacy Practices
Little Oaks Behavioral Services, LLC is committed to protecting health information. This notice explains how we may use and disclose protected health information (“PHI”), your rights, and our responsibilities under HIPAA.
Our responsibilities
- Maintain the privacy and security of PHI.
- Provide this notice and follow the notice currently in effect.
- Notify affected individuals when a breach may have compromised PHI.
- Use or disclose PHI only as permitted by law or authorized in writing.
How we may use and disclose PHI
Treatment
We may use and disclose PHI to provide, coordinate, or manage ABA services and collaborate with other providers involved in care when permitted.
Payment
We may use and disclose PHI to verify benefits, obtain authorization, submit claims, respond to payer requests, and collect amounts owed.
Health care operations
We may use PHI for supervision, training, quality improvement, credentialing, audits, compliance, accreditation, and business administration.
Other permitted or required disclosures
We may disclose PHI to business associates under appropriate agreements; for appointment reminders; to people involved in care when appropriate; as required by law; for public health, safety, abuse or neglect reporting, oversight, judicial or administrative proceedings, law enforcement, workers’ compensation, research, or other purposes permitted by law.
Uses requiring authorization
We generally obtain written authorization for uses or disclosures not otherwise permitted by law, including marketing, sale of PHI, and most uses of psychotherapy notes if applicable. You may revoke an authorization in writing except to the extent already acted upon.
Your rights
- Inspect or obtain a copy of designated records, with limited legal exceptions.
- Request correction or amendment of information.
- Request an accounting of certain disclosures.
- Request restrictions on certain uses or disclosures.
- Request confidential communications in a particular way or location.
- Receive a paper copy of this notice.
- File a complaint without retaliation.
Changes to this notice
Little Oaks may revise this notice. A revised notice may apply to all PHI maintained by the organization and will be made available on the website or upon request.
